I give my approval and consent for the participation of my minor child in the Shots Up AAU Travel program, events and activities, the undersigned acknowledges appreciates and agrees that:

The risk of injury to my child from the activities involved in these programs is significant, including potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce the risk, the risk of serious injury does exist; and,

FOR MYSELF, SPOUSE, MY CHILD’S LEGAL GUARDIAN AND CHILD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASE or others, and assume full responsibility for my child’s participation; and,

I willingly agree to comply with the Shots Up AAU Travel Club stated and customary terms and conditions for participation. If I observe any unusual significant concern in my child’s readiness for participation and/or in the Shots Up AAU Travel Club itself, I will remove my child from participation and bring such attention of the nearest Shots Up AAU Travel Club official immediately; and,

I myself, my spouse, my child’s legal guardian, my child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE the Shots Up AAU Travel Club and their respective officers, affiliates, agents, representatives, successors, sponsors, advertisers and if applicable, owners and leasers of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property incident to my child’s involvement or participation in the Shots Up AAU Travel Club programs, events and activities, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law; and,

I, for myself, my spouse, legal guardian, my child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY INDEMNIFY AND HOLD HARMLESS all the above Releasees from all liability’s incident to my involvement or participation in the Shots Up AAU Travel Club, EVEN IF ARISING FROM THEIR NEGLIGENCE to the fullest extent permitted by law; and,

I certify that my child is physically fit to take part in all Shots Up AAU Travel Club programs, events, and activities; and,

I pledge my child’s compliance to all Shots Up AAU Travel Club rules and understand that my child could be dismissed from the Shots Up AAU Travel Club programs, events and/or activities for any conduct not in the best interests of the Shots Up AAU Travel Club and that no part of my child’s registration fee will be refunded; and,

I authorize any medical evaluation or treatment of my child that may be advised or recommended by the attending physician or emergency medical personnel while participating in the Shots Up AAU Travel Club programs, events, and activities.

Please document for your child’s Coach any allergies and/or medical conditions, including those requiring maintenance of medication (i.e., bee stings, diabetes, asthma, seizure disorders). The purpose of this information is to ensure that medical personnel have timely and accurate pre-existing medical condition information, which may interfere with or alter treatment.

On behalf of myself and my minor children, in consideration of being allowed to participate in a youth sports program provided by the SHOTS UP basketball program including, but not limited to, events and activities, transportation, meals or lodging provided by the Program at or to such events and activities, observation or use of facilities or equipment, participation in or acting as a spectator during any program (collectively ‘Participation'), the undersigned acknowledges, appreciates, and agrees that:

1. I am aware there are risks of exposure to directly or indirectly arising out of, contributed to,by,or resulting from an outbreak of any and all communicable disease, including but not limited to, the virus “severe acute respiratory syndrome coronavirus2(SARS-CoV-2)”, which is responsible for Coronavirus Disease(COVID-19) and/or any mutation or variation thereof,as a result of my or my minor children’s Participation in the Program.
2. I am aware that my Participation or that of my minor children includes a risk of possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19; and that while particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,
3. I acknowledge that incidental contact between participants may occur during my minor child’s Participation in the Program, even with physical distancing protocols in place; and
4. . I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my Participation and the Participation of any of my minor children; and,
5. I willingly agree, on behalf of myself and any of my minor children, to comply with the stated,reasonable, and/or customary terms and conditions related to my Participation—and that of my minor children—as regards protection against infectious diseases; and if I observe any unusual or significant hazard during my presence or participation, I will remove myself and my minor children, as appropriate,from Participation and bring such hazard to the attention of the nearest official immediately; this includes but is not limited to taking temperature of my child prior to each session that he/she is expected to participate in and withhold participation if there is any sign of illness or knowledge of contact with infected people and,
6. I, for myself and on behalf of my heirs, assigns, personal representatives, children and next of kin (‘Releasors’), HEREBY RELEASE AND HOLD HARMLESS the Shots Up Basketball, a not-for-profit corporation, it officers, officials, agents, consultants, and/or employees, other participants, volunteers, sponsoring agencies, sponsors, advertisers,and if applicable, owners and lessors of premises used to conduct the event(‘Releasees’),WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

This is to certify that I, as parent/guardian with legal responsibility for the minor child participant.
